SUD Case Management

Case management provides coordinated individualized and goal-oriented support, focusing on improved
self-sufficiency to Substance Use Disorder clients, as clinically indicated, through a comprehensive need
assessment. Activities are provided to assist and support individuals in gaining access to needed
medical, social, educational, and other services essential to meeting basic human needs.
